experience letter format?

I am trying to obtain an experience letter from my formal employer. I wonder whether there is any format/template for such a letter and what information should be included.

should include

While I don't have a format, it should include

1) Exact dates worked
2) Kind of work done
3) Should be on a letterhead
4) Contact details of the person giving you the letter

by the way, do I need ask previous employer for two copies and send one original letter Or just ask for one copy and send a photo copy to INS(generally, we are not required to send original)?

I would suggest you to include partially or fully the same language (the same wording related to your work profile) what is being given in your labor certificate or in the newspaper advertisement. This will help you a lot.

if you get two copies its better, just in case you lose or damage one or for whatever reason, just send copies, no originals are required.

Suggest you scan and keep them online somewhere as well :-)
