ETA 9089 can you please check it

I have a question regarding ETA 9089. Yesterday my laywer sent me a copy of ETA that he is going to submit tomorrow. After I read a lot of posts here, I have a few concerns if everything is correct in the application. I would appreciate if someone can share his/her opinion. Thank you in advance. (this is for EB-3)

They are filling those forms out based on all of the recruitment that has already been done. So, if you want to change any of those, you will have to start all over, submit a new prevailing wage, etc.

I think the issue over here was if the answers are logic. BTW is there any website/book where someone can find a sample filled ETA? Just asking. We hear DOL rejects a lot of ETAs so where's the main problem? Forms are prepared by the attorneys and I don't understand why so many of them are not approved.
