ETA 750 or ETA 9089

I am preparing the documents for I-140. My previous lawyer sent me the Labor certificate - I am not using the lawyer anymore.

On the list of things to submit, it says ETA 9089. My labor certificate form says ETA 750? It has two pages on double sides. It came with the Final determination letter but no ETA 9089. Can someone tell me why? :confused:
 
I just checked the I-140 application and couldn't find anything there about 9089. Also checked the ETA9089 form itself and it looks like it is used when you are trying to port a PD from an ealrier labor approval (earlier 750).

So when you get your labor certificate, did you receive ETA 8098 or ETA 750? The 750 was filed and certified by DOL and that's all I have. I don't remember filing 8089 or recieving it. I am just really confused.

I found an online lawyer (do it your self kit) and in the instruction, it says I need to submit ETA 8089 along with I-140 form. :confused: :confused:

Dont worry. I also only have an ETA750. Mine and yours are labor cases approved from Backlog centers. The new PERM system approvals are ETA 9089 whereas the old system (or backlog approvals) are ETA750. Check this out -
