Entering US with only re-entry permit approval

I am in a desperate situation. I have a GC and I left USA Nov 2003 with my family as my husband's company had sent him to India on a temporary assignment. I applied for my re-entry permit before leaving the USA. I gave my brother's address in USA for the permit. We waited for more than a year to check the status of the permit. When we checked, found out that uscis had already sent the approval and the document in dec 2003 and I didn't receive them. Even the post office didn't return anything to uscis.
We are supposed to return to US this summer. What can I do now? When I
spoke to the immigration officer in Nebraska, she said that I should get a duplicate copy of the approval. With that I should get a travel document from the local consulate and then enter USA. Is that going to work?
From what I read, the travel document and the approval may not be enough to prove that I have the intention of maintaining GC.
My husband and kids are US citizens. I have US bank account, credit cards,
driver's license, mutual funds, US address. I filed taxes in the US in 2004 and we will do it this year also . I also have a social security number.
Any help/suggestions is appreciated.
 
That travel document plus the receipt of your re-entry permit application should be good enough for re-entry. The receipt is the proof that you appiled for re-entry permit before departure.

Normally for this kind of situation it's always better to advise USCIS to send the approved re-entry permit at the nearest consulate (or attorney). The mailman probably did not deliver that mail assuming the mail is sensitive immigration related document and the address is not your address (but your brother's address).
