I just want to make sure, if i should mail back the checklist form to my country US Embassy. I just mail back my DS230 Part 1&2, DSP122, and photo to Kentucky Consular Center, but there's another form, which has the Kuala Lumpur(My country) US Embassy address on the top right hand corner, should i mail that form to my country US Embassy? Besides can i know when is the due date to send this form back to KL US Embassy? Please advise Thanks!!!
