How to get an emergency appointment in chennai consulate for H1B renewal for Indian citizen residing in USA with Visa stamping expired more than 12 months ago. Have a valid H1B approval notice valid until 2009.

Is there any specific letter format needed from my employer? If yes, what is the format?
