No, EB1-EA does not require any labor waiver application.
Yes you can fill and submit I-140 form online and then mail all the evidence through post along with the online receipt. (you don't have to mail the I-140 application form again with your documents).

Yes, you need to separate all the evidence according to the category e.g.,
appendix 1 - Resume and Certificates
appendix 2 - awards and scholarships
and so on....
Once you draft your cover letter and mention about your achievements, you should refer to each apendix #. e.g. my work has been cited by....(please see appendix #)
