EB-2 evidence of employment

Hi,
I would like to start to collect evidence of employment for EB-2 category.

1)If I can not get a reference letter from a past employer, would a record of employment do?
2)Do these reference letters need to be dated within a certain time? (ie within last 6 months). Can I use a reference letter given to me 10 years ago?
3)If I have a copy of a reference letter, would this work?
4)What are the bare minimums of content in a reference letter?

Try to get affidavits from previous co-workers who were together with you at work place and provide details about about your job title/duties and job dates.
