EAD Supporting Docs list??

Can someone please post the list of supporting docs that i need to submit with my wife's EAD application. We have received our I-485 receipts few months ago.

--signature card: 2
--I-765 application

--i-485 receipt
--latest I-94

Is the signature card still needed? On http://www.ins.gov/graphics/formsfee/forms/i-765.htm

there is special instruction:

Please note: The blue "Signature Card" mentioned in the form's instructions is no longer required by INS.

sounds like the signature card is not needed any more. Can some one confirm?
