looks like that i may have to go to local INS for 3rd EAD!! (after 90 days)
as far as i know
i need
1. filing papers
2. ead receipt notice
3. old ead card
the photos are not necessary as they will be taken there..
When did you apply for your EAD extension. What is the receipt date on it?
I applied for my EAD extension in July, received the receipt notice. Good luck
