EAD renewal Questions :Pls help

Planning to apply for 3rd EAD for me and spouse myself rather than paying 250 dollars to the lawyer

I need help for filling the form(from Columbus, OH)

Step 11:
Have you ever before applied for employment authorization from INS................If Yes we have to fill Which INS Office and Date:
INS Office I put : Nebraska Service Center

For Date what should I enter? Should I enter Receipt Date of previous EAD or approval Date of prev EAD?

Step 14:
Manner of Last Entry (Visitor, Student, etc.):
I went to my country on Advance Parole
Should I put "Advance Parole"

Step 15:
Current Immigration Status (Visitor, Student, etc.:
Should I put "i-485 Pending" or something else?

Step 16:
I am putting (c) (9) ()

What should I put in third () ?


Where should I sent it to
Shall I file it in NSC I am in Columbus, OH) , i.e.
I-765 P.O. Box 87765, Lincoln, NE 68501-7765 ?

14. Entered on Advance Parole(I-512, DA) --refer your I-94
15. AOS(I-485 Pending)
16. (C)(9)()- nothing for third bracket

submit it to where ever your case is pending, service center or local INS office where your case is transferred to.

You should be fine.
