EAD Renewal Question --- Pl. Help

Hi:

I am ready to apply for my 3rd EAD and have a few questions.
I am sending in the following.. I was wondering if we need to send a copy of my I-94.. Is a copy a I-94 necessary.. Please respond soon.

1. Check to USCIS for $120
2. 2 Photographs
3. Filled I-765 Form
4. EAD Mailer
5. I-485 Copy
6. Copies of current & previous EAD
7. Drivers License copy.

Copy of I-94 is necessary. Otherwise you are good to go.

Also, better to include
a. Passport Photo page copy
b. I-140 copy.
