Typed I-765 form (you will get the writable form from immihelp.com). 2. $120 cheque fvg Immigration and Naturalization Service. 3. Two photographs taken within past 30 days. 4. Copy of the receipt of the filed I-485 application. 5. Copy of the latest I-94 card (front and back side) 6. Copy of the latest EAD card (front and back side) 7. Perforating Stub on the EAD card sent by INS. Hope this helps. Good luck to you.
