EAD renewal application question - please advice

Hi

I am trying to renew my EAD. I already got one EAD. There is one item 11. in the form Did you apply for EAD before? Where ? and Date(s).

My question is what date I should give here?. Is it the date my EAD approved or EAD receipt date?.
