EAD renewal after expiry

I have to renew my wife's EAD. Her EAD is exipired about six months back. She is currently not working anywhere.

Questions:

1. Do I have to apply for new EAD or for renewal?
2. Which form to use?

A: You don't have to if she does not intend to work. If you ever want to get her a new EAD card, file "renewal".

2. Which form to use?

A: I-765.

when filling for you wife, r u doing yourself or by an attorney...
if you yourself then can i know what is appropriate for question 10 and question16

question 10: regrading A# or I94 which one should I write
Question 16: what is the category for H1 visa and H4 visa

I did the same thing for my spouse's EAD renewal in April.
All you need to do:
1, I765 form(download from BCIS web site)
2, two photos(write name & A# on back)
3, A paycheck to BCIS
4, copies of both side of old EAD card
5, copy of I495 receipt
6 copy of current status approval notice(H1 or H4...)
7. copy of passport and I94
7. A simple cover letter
