EAD renew question 14 and 15

I have to renew my EAd now. I try to do it myself. I don't know how to fill question 14 and 15. If anyone have experiences, please help.
Question 14: Manner of last entry:
Question 15: Current Immigration Status:

I entered US using AP and I was H1-B before AP.
