EAD (I-765) Question 11

Hi, I am a bit confused about Question 11 ("Have you ever applied for employment authorization from USCIS?")

Here is my situation:

- Currently on H1-B and have been for nearly the past 3 years.
- Concurrently filing my EAD (I-765) with my I-I30 (Marriage based immigration) & I-485.
- I did apply and was approved for EAD BEFORE I got my H1-B. (I was on F-1, Student visa, and applied for EAD for my Optional Practical Training).

Should I check "Yes" or "No" for question 11? Any suggestions?

Check YES and give details about your most recent EAD application (OPT is still an EAD - just a different classification).

Then at the very top of the form, should i check the "Permission to accept employment" box or the "renewal of EAD" box?
