EAD Filling/I-765 Form Questions

I was hoping some of you could advise me regarding the EAD/I-765 form. Here are my questions:

What should I enter in line "15. Current Immigrant Status"?

I am on an H1B, but I am also waiting for Adjustment to Permanent Resident Status. So should I enter "H1B" or "Waiting for Adjustment to Permanent Resident Status"?

Also, I wanted to verify that my list is complete:
- 2 EAD Style Pictures
- Copy of I-485 Receipt Notice (Notice of Action)
- Fee ($120 Check)
- Copy of I-94 (both sides)
- Cover letter (does anyone have an example cover letter?)
Should I include anything else?

I would appreciate your help very much!!

It has all you want. And you have the right answer to the question 15 concerning your visa + status.

Ans: H1B Temporary Worker.
(this is what I mentioned when applying for EAD)

Ans: Also include the foll:
1) Copy of your EAD (front and back)
2) Copy of your EAD jacket (letter which came along with EAD)

Cover letter is optional.

This is my initial application for EAD not a renewal so I couldn't enclose a copy of an old/expired EAD. For line 15 I ended up putting "HIB (Adjustment of Status pending)" - lets see if that is okay with the INS. I will update my posting in the unlikely event that it wasn't correct. I also included a cover letter but I only stated that I am applying for an EAD and then listed all the enclosed documents. I closed with a sentence about contacting me in case they have questions or anything is missing and gave phone/email in addition to address. It is really a fairly simple application and not much to it (if I did it right that is) and the lawyer would have charged $500. ;)
