EAD Filing Online

My spouse is typing all the EAD info online and has the following question:

1) During OPT , worked on EAD for 2 months but it was from INS. Now one of the quesion is
Have you ever before applied for employment authorization from BCIS?

though it was from INS and not BCIS, should we mark it as Yes ?

2) For (c)(9) eligibility status only
Please select the location where your
I-485 is pending:

since all the forms are going to CHICAGO LOCK BOX, should I select HOUSTON DO or CHICAGO DO ? Please advise ASP..Thanks.
 
I don't know about your first question, sorry.
But your I-485 is being processed by your local DO. So if your local DO is Houston, then yes, that is what you should select.

USCIS was called INS earlier and BCIS prior to that. They are different names used in the past for the same agency. You could mention the OPT EAD for q.11 if you are mailing the form

I never filed EAD online so i am guessing the answer for Q2. I think you selected yes to the previous question and the software thinks that you have an I-485 filed and are filing a renewal EAD and hence it is asking for the DO where your I-485 is pending. Try selecting No for the previous question and i think you will not be asked this question.

One cannot file EAD till the I-485 is filed. As for filing it online, there is not much advantage over filing it along with the AOS package. Filing online saves only 3-4 days and the processing time is the same as other EAD. If you ask my 25 cents, you should file it with your AOS package.

It's Asking for Proof of doc if we answer yes. My husband is missing his old OPT EAD card( 5 YEARS old) but I found the old EAD approval letter. would that be sufficent to attach ?

For question 16 in the EAD form[ go to part2 of the instruction, eligibility categories. place the letter and number of the category u selected from the instruction
For family based, it should (C)(9) RIGHT ?


Also, have the following question in the AP: iN Part2 APLICATION TYPE:
my husbaand must select option D right ?
D. I am applying for AP document to allow me to return to US after temporary Foreign travel.
