EAD extension documents

Do I need anything else.

1) I-94 ( stamped as June 3 2003 when used AP )
Is copy enough
2) EAD card copy.

3) I-485 reciept copy.

4) photos ( ADIT style)

Also, is the approval mailed on the address
we state on the EAD form or is it what we had given on I-485
application.

I have moved since I-485 filling.

Can I file myself or do I need a lawyer ?

Have used AC21 and current lawyer is
of previous comapny and wont file extension
or take money to file without previous companies approval.

Do we need other documents in case of RFE like I-140 or
Labor?

I am taking my chance ... How did you USE AC21 .. I am in a kind of state where I wud have no option but to do the same ..

Sounds like Using AC@! takes a permission from authorities

Is it so ? and Is it possible that the previous employer may harm your I485 approval process ? after 180 days of concurrent filling ?

I filed myself last week .
I sent
1> Current EAD card copy & the original tear-off portion of current EAD
2> 485 receipt notice copy
3> Photos & fees

I didn't send copy of my I-94, anyways since the stamped date is expired.
