EAD Extension Check list..

What are the list of docs to be sent along with I-765. Is it I-765 or any other form? In the wake of the recent tragedy and with the delays in approvals almost certain, is anyone expediting sending their EAD extensions? Please respond..

Enclose the following with your application for EAD (I-765)
1. Copy of your current EAD card
2. Top portion of your EAD document
3. Copy of your I-485 receipt notice
4. Photograph\'s
5. A check for USD 100.00 towards fee

EAD top portion

When you receive your EAD card, it is attached to a approval notice. You will find a perforated section at top of this notice, where it says that \'to speed up the renewal, please detach this section\' and send it with renewal application.
