EAD Documents List please ....

I plan to apply for EAD on my own ... my lawyer is a freaking nut case.
As far as I know the documents needed are
1.I765 signature card (where do i get this ?)
2.copy of I94
3.2 photo\'s

what other documents do i need to attach ? example should i attach the approved 140 application.
Any help would be appreciated.

You will also need a copy of the 485 receipt notice and a Check/MO (preferably MO) for $100.
You can get the I-765 form from the INS website by mail(takes about 10 days). It will have a blank signature card that you can use.
You can also read the instructions that come with the 765 form to make sure you have everything.
 
485 receipt notice ????

I plan to apply for this concurrently ....In this case i would not have a 485 receipt .... would my i140 approval notice be fine?????.
