EAD Documentation for renewal

Could any of you direct me how to write the cover letter for EAD renewal.

Are these enough or do I need to send any more documents for EAD renewal ?

1) Photos – 2 (Not earlier than 30 days) – Write name and “A # “ with pencil on back side of Photos
2) $ 175 Filing fee payable to “Immigration and Naturalization Service”
3) Cover letter
4) I- 765 Filled in and signed form
5) I- 485 Receipt notice copy
6) I- 94 – Copy (Front and Back)
7) Last EAD - Copy (Front and Back)
8) Marriage certificate for spouse
9) Mailer top off slip which came with the Previous EAD
 
You can use the following sample cover letter for EAD for you and your spouse ( This has the list of docs required also) . I would prefer colored copies of Driver License AND Passport ( First Page).

I have applied to Adjust Status to permanent resident per application EAC-XXX with you. Please find enclosed my application for renewal of EAD along with the following documents:
1. A personal check for filing fee for an amount of $175.
2. An envelope with two photos ( Write ur name,DOB and A# with pencil).
3. Form I-765 duly filled.
4. A copy of the last EAD card (Front and Back)
5. Cut out mailer of previous EAD Approval
6. A Copy of Drivers License
7. A Copy of Passport (which has Bio-graphic data)
8. A copy of my H1 visa stamping.
9. A copy of the last I-765 Receipt Notice.
10. A copy of form I-485 receipt notice (EAC-xxx).
11. A copy of my husband’s I 485-receipt notice (EAC-xxx) << Only in case of your spouse
12. A copy of my husband’s I 140 approval notice (EAC-xxx) << (For urself remove 'husband' ;-))
13. A copy of my marriage certificate <<Only in case of your spouse
14. A copy of I-94 departure card (Front and Back).

Kindly approve my EAD renewal.

V.IMP about check payable too

HI,
We need to make the check Paid to 'U.S. Citizenship & Immigration' serivce but not to INS as u specified. INS is gone. I sent the check payable tothe above and got cleared in 5 days of sending and also checked this by calling 800 before sending my package.
 
Did you put your and wife's application in seperate envelopes
with sepearte cheques or did you just put everything in one envelope
with one cheque??

Should the check be payable to USCIS?

are you sure about the check payable to is "USCIS"?

are u sure we need to send copies of DL and passport?

It is better to send color copies of DL and passport biographic page, unless you want them to issue an RFE. I had sent it and my app got approved in little over a month. Hope it helps!
