EAD attained through spouse, got medical degree from abroad

Hi everyone
I hope this question has not been answered before..my wife has a medical degree from outside the US, she is now preparing for her USMLE..she is hoping to start some voluntary work soon...however, I am not sure how that will work out. I am in MA...any suggestions
