EAD Application Process for L2 Visa

Hi,

I am on L1A Visa and my spouse is on L2. I was trying to fill up the EAD application form for my spouse and had confusion in 2 points. Would anyone help me with these answers ...

1. Manner of Last Entry (Visitor, Student, etc.)
2. Immigration Status (Visitor, Student, etc.).

We are living in Milpitas, California. Where do I need to submit the form ?
