EAD Application: Date of last entry to USA

HI All
I return to US from Canada in July 2002 but my I-94 states Jan-2002 since I did not surrender my I-94. On the EAD application, which date should I enter as my last date of entry? I need to submit a copy of I-94 as support. Anyone in similar position? Please help

Did INS stamp your passport when you last entered in USA ?
If not then date on I-94 as this is the actual recorded date by the INS, you can attach a copy of I94 as a proof.
