EAD / AP Renewal

Has anyone applied for EAD and AP renewal on own?
What all documents are needed? Are any or all of the below needed?

1. Copy of Social Security Card
2. Copy of Drivers License
3. Copy latest PayStub
4. Letter for employment verification
5. Signature card

BTW, 485 RD 11/06/2001 and ND 11/28/2001

Download Form I-765 from ins.gov. Instructions are pretty clear. Provide the docs they ask for, plus a copy of your passport biographical page. Signature cards, I believe, are no longer required. However, we did attach one when we applied in April.
