EAD/AP renewal

I gto GC in Dec2004. I am applying for my wife EAD/AP renewal. I have few questions

1. I cannot type in 131 on my system? do I need to do anythinhg for this? or can it be handwritten?
2. what is field 16. category in 765 form?
3. as it is renewal, is it granted and dates?
dates? means approvaldate/RD/ND of last EAD?

Please let me know.

Try to install Adobe Acrobat Reader latest version and try

for field 16, type C(09)

i think you are asking item 11 in I-765, say Granted and put the approval date of the current EAD

Thanks for the info.

I installed adobe reader 7.0 but of no help. Can I fill it by hand?

Do I need to attach a supplement letter stating that why I need renewal of AP? Do I need to send a copy of my GC and 485 approval notice? If i send there is no harm, right?

To which address, Shall i mail these docs? Vermont/ Chicago?
this is for my wife's AP renewal. My GC is based on Employment based? I think my wife's also based on it.
