EAD and AP Check List

Guys,

I am getting ready to file for EAD and AP. Here are the documents that I prepared.

I-131 Form filled in
I-765 Form filled in
Personal Check payable to USCIS $170
Personal Check payable to USCIS $180
copy of 485 Receipt
copy of I-94 (front and back)
4 photographs
copy of drivers license

I have this ready for both me and my wife. Can you guys let me know, if I need anything else?
I read somewhere I need a letter explaining need of travel for AP? Is that true or 485 receipt is enough?
Can I include both my application and my wife's application in the same envelope?
Do I need to include a cover letter listing the contents of the envelope?

Any help is greatly appreciated.

Here is what I did. Mailed four applications in one envelop. Each application was bound togather by paper clips with a color cover sheet listing applicant's name, A#, type of application and documents included.

Document lists are: addtional docuement for dependent in ( )
I765
1. Filing fee: check in the amount of $180.00
2. Two passport-style photos
3. Form I-765
4. Copy of I-485 filing receipt notice.
5. (Copy of I-485 filing receipt notice for primary applicant.)
6. (Copy of I-140 approval notice for primary applicant.)
7. Copies of I-94 (front and back) and latest H1B (H4) approval notice (front and back), if there is extension after last enrty.
8 Color copy of driver license.
9. Color copy of biographic pages in passport.
10. Copy of birth certificate.
11. (Copy of marriage certificate.)

OPT (EAD) copy

If you were a student (MS or PhD) and have an approved EAD card, send a copy of that too. In addition, you have to fill the items that ask you about previous EAD(s).
