DV lottery , Green card Forms Fill? form DS-230,and photos

I was selected to DV lottery , and i have several questions:

1.i have 2 children under 10 ,do i need to fill separate forms for each one of them includes my wife? so at the end i should fill Form DSP-122 only for me , and other 4 times Form DS-230 for each one of us separately ?

2.how to attach our photos ? should i attach them by glue to DS-230 or not?

3.should i write my case id on the all the papers ?and what to write on my family papers?

4.Form DS-230 ,Question 39 , Name and Address of Petitioner ? is that me ?the one who won the lottery ?

5.Form DS-230 ,Question 14,Spouse's Maiden or Family Name?

my wife Maiden name/Family name before marriage was "Shamut" ,and after marriage it become as mine "Simri" , what i should write?

5. I think you can write there both..but that I am not sure..

1) Fill separate forms for all dependents (you will have to indicate your wife's and children's details on your form as they are dependant on you).

2) DO NOT glue the photographs. Firstly ensure they are as per the guidelines on the DV site and simply paper clip them on your forms.

3) DO NOT write anything else on the forms except for answering the questions in their respective fields.

4) This question does create some confusion but to my knowledge the petitioner is yourself (the person who was selected) or someone who is petitioning for you in the US i.e. your host where you will stay when you enter the US so you'd need an I-134 form filled out by that person. I would say you should be a self-petitioner and have sufficient finances/savings to present at the interview and you'll be fine and won't have to go through the whole I-134 process.

5) Spouse's name - put exactly what is on your wife's passport with regards to family name. The DV visa will be issued as per your passport details.

Guys, I have one question regarding form DS230, Part I, on the page 2..
As I decided not to mention about my wife (because she is on J1 visa now), so point 31a I will leave blank..
But I would like to ask regarding point 31B, "who will follow you at a later date", shall I leave it blank too, in order not to affect my wife's ability to finish her training as she cant get waiver yet, or if I leave it blank she wont be ever able to accompany me even after her waiver?
