can anyone explain what this form is and when it needs to be filled out?
Are you sure you have written that down right? DV 2001 was the diversity visa for the year 2001, I have never heard of any such form with the same name.
you mean ds-2001, "Applicant readiness for interview" form not dv-2001.
You need to download it from the embassy where you want to have the interview. DS-2001 is dependant on your national American embassy.

That's interesting. I did have to fill in a couple of consulate specific forms (which they sent me) in preparation for my interview. But neither of them was the ds-2001. I guess this is one of the things that varies a lot between consulates.
