DSP-122 help!

Hi!
I got the notification letter for DV2009 winner. However I did not find in the package of documents any DSP-122 form (only DS-230). Since it says clearly in the instructions that I have to complete the DSP-122, I tried to download a copy from the internet, but I noticed that the form has an expiry date12/31/2006 in the top right corner.

Does that mean the form I retrieved online is invalid?

If so, is there a way I can find an updated version?

thanks bro but that form is already expired (EXPIRATION DATE: 12/31/2006).

correct me plz if i'm wrong. thanks again.
 
Hey Guys

If you go online on DV program you will find new D 122 ............you do not have to sent the one they send you .just download and make it copy in one page instead of two pages .

I hope that help,i did that too.

u mean kcc accepted that form also even if the have EXPIRATION DATE 12-31-2006 7 ALSO IN 1 PAGE INSTADE OF 2 PAGES. PLEASE GIVE ME THE LINK TO DOWNLOAD THAT IF U CN
