Please clarify the following questions. I appreciate your early reply.

Form DS230 (part I & II)
=================
1. I 've got big name with 5 words including lastname, since my name has got 4 words, all H1's has only 3 & first letter from 4 word as middle intial,
- Can I put 3,4 words as first name and 1,2 words as middle name ?

2. Question 10 on DS230 part I
- I've been living in US for 4+ years with wife & kid(born in US): Do have to give US address and needs to specify both (wife&kids) names ?

3. Question 11 on DS230 Part I
- Same address as in Ques 10, so I can put 'N/A' ?

4. Question 13 on DS230 Part I
- Do I need to give the same present US address ?
- Where should I give India address to get Packet 4 to india address ?
- what should I do to get Packet 4 to US address ?
- What is the best approach ?

5. Question 19 on DS230 Part I
- We had US born kid, Do I have to add kids details, even he is US citizen ?

6. Question 20 on DS230 part I
- Can I just guess(best) the month & Year where I lived
- Do I need any supporting documents for the dates ?

7. Question 21a on DS230 Part I
- Do I need to mention my kid's(Born in US) name along with wifes name ?

8. Question 25 on DS230 Part I
- I visited india once during 4 + years, lived in same state but different cities: Can I just mention date of entry into US first time to present and location is the name of the state or do I need to give details about each city in the state where I lived & duration ?
- what is "A" NO ? (I am on H1B - does it applies to me)


9. Question 29 on DS230 part II
- Petitioner means my Employer name and address right ?

10. Question 33a on DS230 Part II
- I have an SSN, Do I need to apply for NEW SSN ?

{What format did you use in the passport, your driving license
and the SS Card? }

My passport has

Family has : last name 1, 2 word
Given name : 3, 4 word



Drivers lic has : 3 4 -1 -2 lastname

i-140 : last name, 3 4 first letter from 1 word

SSN : same as drivers lic (do not have it right now)

Answer: One DS 230 for each GC applicant including primary and derivative. No need to write up a ds 230 for children who are US Citizens. Text box 10 should ideally be your current address which should be a permanent address until you get your physcial GC. Yes include your wife and kids name too.

Answer: Same as Text 10 unless you want your welcome letter and physical card to be mailed elsewhere.

Answer: YEs

Answer: The best option is to put all usa addresses wherever asked and then when your interview is scheduled you can email email / call the consulate and have them mail it to your indian address. This way you get it at usa address as well as indian address.

Answer: Yes.

6. Question 20 on DS230 part I
- Can I just guess(best) the month & Year where I lived

Answer: Yes if you do nto know the exact dates. Definitely a guess such as May 2001 instead of June 2001 is a good guess but May 2001 vs May 2002 is a bad guess.

Answer: Not necessary. If you have that is good else dont stress about it.

7. Question 21a on DS230 Part I
- Do I need to mention my kid's(Born in US) name along with wifes name ?

Answer: Yes if they plan to come back with you. Mention your childs name and in brackets put USA citizens as a note.

8. Question 25 on DS230 Part I

Answer: A# number is Alien number which you will get on GC approval. So leave that blank. Line 1 enter first entry date into usa - first departure date, port of entry and type of visa. On Line 2 enter second entry date into usa - till current, port of entry and type of visa. If you came to usa on lets say F-1 and then changed to H-1 then state there too in the proper timeline.

Answer: Yes.

10. Question 33a on DS230 Part II
- I have an SSN, Do I need to apply for NEW SSN ?

Answer: Yes. After your CP approval, you need to get a new SSN card which will have the same number but without the work restriction legend that you have from USCIS/ INS today. And your dependent spouse will also state yes and she will get a new SSN number with a card on returning to usa with I-551 approval stamp at port of entry.

{Since you would be filling a seperate form for yourself and
your wife, you would repeat the same informatin in both the
forms}

{Write 'Same as Item 10' or repeat the address }

{You would have only one present address, right?}

{Normally this is given later to the consulate by email }

{Do not provide an address in India}

{Get it in India}

{It does not. But mention that you are on H1 and give the
Visa number}

{Yes. You will be given a new SS card with the dependency
on work authorization taken out (you no longer need a
H1 to work in US) }

Location you mean port of Entry like, chicago, detroit, etc, not the city where I live ?

A small clarification...answer to 25 on ds 230 part could be POE or place of residence in USA. In my case, my POE and place of reisdence are same so it was not an issue.
 
question about DS-230 form

Hi,

I am trying to fill DS-230 for my mother and father and have question regarding 21a and 21b

21a. Person(s) named in 14 and 19 who will accompany you to the United States now.

should I metion my mother name which i mentioned in 14 as spouse eventhough I will have seperate DS-230 for mother?

21b. Person(s) named in 14 and 19 who will follow you to the United States at a later date.

I have also applied for my brother (unmarried) and sister (married) and their application has been approved but need to wait until priority date become current.
should I mention their name in 21b


I will appriciate prompt response

{Each will contain the other person's name and details. Your
father and mother will have their respective Form DS-230.

I am assuming that they share the same case number. That
is the case under employment category IVs. I do not know
how it is for Family based cases}

21b. Person(s) named in 14 and 19 who will follow you to the United States at a later date.

{This only applies if several persons are appearing for the
interview at the same time and some of them are travelling
at a later date. If that is not the case then you should just
say 'Not Applicable' or 'NONE'. }

Also, my mother and father both as different case number. They are not sharing one case number.
 
DS-230 Part I

Hi,
I have more questions regarding DS-230 Part I, which Iam trying to fill this form for my parents
Question 16 to 18
My father does not know the birth date of his father and mother. Is it ok to guess?
Also, all the information you provide in this form need to prove?

Also, address you provide for applicant (Question 13) should match with the passport?
I am not sure if US consulate in Bomby matches the passport addres with the form. (My father is living in the same address but the appartment number has been changed).

One more questions. My father is going to use me as his agent (in choice of agent address form).
As a agent I will fill the DS-230 form and the question is can i sign the from behalf of him. The form says SIGNATURE OF APPLICANT.

If My parents has to sign the from then can I download the form from web and fill the the form and send it to them to sign. That way I will have the form ready the time NVC ask for it.
OR I should use the form which NVC will send me.

Here I go again with questions

Question 25 what occupation do you inted to work in United state?
Can i write any thing? since we are not sure about the job

Also, as I metioned priviously about my father's home address and wrould like to clear one more time is that when I submitted the form I-130, I wrote the address where he was living (e.g. B/4 Akshardeep Apt) but the passport had old address (B/1 Akshardeep Apts )the building is the same but the appartment number is different.
Do you know if CP in Mumbai will give question about that? Should my father get address change in his passport?

Answer: It is advised to write the profession your GC is filed under. So if your GC is based on IT/ Computers then write IT/ Computers and so on.

Answer: It should not be a problem. One should generally put current residential address. I had a different address in passport and then my parents moved into another house so in my form I have the new address although the old one showed up in the passport. I got the new address endorsed in my passport later!

{kd may have thought this DS 230 is for you.

But this being a family sponsored case then I am not sure
how they fill this in. In my opinion saying anything like
'Any suitable occupation' or 'retired' should be OK.

If he has retired in some particular occupation then
mentioning that would sound logical too!! }

changing their residences all the time. }
