One more question for clarification :

In part 10 and 11 where it says Permanent address in United States where you intend to live and Address in United States where you want your Permanent Resident Card( Green Card) these two questions also ask to include the name of a person who currently lives there.

I am in USA . Do I need to give my own name. Also I and not sure where I will be living permanently should I give them my current address as I intend to live there for a while.

Include your name with the address.

"permanent" does not mean for the rest of your life. In this case it means that you do not have a definite plan to move from that address. If you are planning to move in 2-3 months, give another address e.g. a friend's.
