I understand for item # 29 (Name and Address of Petitioner), we need to provide employer's name an address for primary applicant. In case of derivative applicants (for dependents), what details should we provide (Employer's or primary applicant's) ?


Regarding item # 33a (Have your ever applied for SSN: Do you want SSA to assign you an SSN or issue you a new card) -- My Spouse and I already have SSNs -- should I request for new SSN card? What is the difference between existing SSN card and new one?


Regarding Blue Dart DD: For four (4) applications (passports); Single DD or seperate DDs for each applicant?

If signle, what should be the DD amount ((Rs. 300 [courier fee] + Rs. 50 [Insurance Premium] = 350 ) = 1400 or 1250)?

For derivative applicants provide principal applicant's details under "petitioner".

Indicate that you already have SSNs. You can either request a new SSN card (the new card will not have any restrictive phrase like "valid for employment only with INS/BCIS authorization..") or you can do so separately after you land in US and apply for SSN cards at local SSA office. SS numbers will remain unchanged.

A single DD is sufficient for all applicants.

For 4 applicants I think it is Rs 300+4*50=Rs. 500. Courier fees are 300 and 50 is for each passport/visa packet. For a single applicant it will be 300 +50=350. You will receive instructions with this info in detail along with your packet4.
