DS 230 Part 1 question

Hi All CP GURUS,
I received my packet 3 docs from NVC and while filling the forms out ,I got the follow questions/doubts. Refer to http://travel.state.gov/DS-0230.pdf for the form:

1)Question #11(Address in United States):
Can I put-> Same as item #10

2)Question #12 (Your present occupation):
I currently work for company A and my occupation is senior s/w engineer. I am doing GC via company B where they have put my occupation in LCA as "Computer Programmer/Analyst".
What you guys think I should write? Coz its 'occupation' & not 'title' so there is a thin line I guess...

3)Question#22(List all employments for last 10 yrs)->
Can I write only US employments? Currently I don't have my India employment records like paystubs,tax statements etc as I only worked for 6 mths in India before coming here. Do I still need to write my indian employment details? I only have my expereince letter from indian employer with me here. Please suggest.

4)Question #23 (List all educational institutions attended) ->
There is a column which says "degree or diploma". For my MCA & BSC I can write 'degree' but what about my 'senior school' & 'secondary school'? What am I suppose to write for that ?

5)Question #25(List dates of ur previous visits to USA)->
I came on H4 and then started working on H1B in Florida & then moved to California on H1B itself. My question is, in the "Location" column ,do I need to specify which state I resided for how long or I can just put USA?I am specifying my period of stay in USA in H4 status as well as H1B sttaus but do I also need to specify what period I stayed on H1B in Florida and what period on H1B in CA or I can just put USA as location and have only one entry for my entire H1B period?

Lastly,the package doesn't have the DS-2001 form which I saw in immihelp.com. Do I need to send it or its not required when you go via NVC route? NVC only asked to send DS-230 Part 1 & the page which has my case no. barcoded on it. Maybe DS-2001 is only required for AC 140. Please Clarify!

{A short title for what you do. Ex: Java Programmer}

{You have to mention all your employment history going back 10 years . It does not matter even if you don't have pay stubs etc. }

{It is not necessary to go back to your school records. Just
mention your Bachelors and MCA }

{You have to put all the places you lived with the respective
VISA number against the entry and the period. Use an
extra sheet if required. }

{DS-2001 is no longer required. It was a requirement some time
back. Just follow the instructions as given in the Packet 3}

Just a quick recheck on 3 questions:

Question #12 (Your present occupation):
Can I write "Computer programmer" instead of "JAVA/VB/ASP Programmer"?

Question#22(List all employments for last 10 yrs):
Since I don't have any paystubs or tax docs for my indian 10 mth job ,is there any possibility than the officer can ask for those docs?
Coz even my parents couldn't find my older paystubs-they seem to be misplaced. Please advise.

{It should be OK. They may not ask you to qualify that}

{I don't see the possibility of them asking for this information.
It is my suggestion to include this employment info, because in
all probability this 10 month period would have been
considered as your experience at the time of filing the I-140.}

{I am sure mentioning just the degree and post-graduation
is sufficient. That is what I did. On the other hand there is no
harm if you mention your school information. In such case just
mention the name of the qualifying exam.

Ex: Intermediate, 10+2 , SSC and so on. }

Please suggest
