I am preparing DS 230 forms for my parents.
I have a question Regarding Qs 19 - Family Name
My father's passport only have the first and middle name of his father - No last name
His school certificate have Fisrt, Middle & Last Name of his father. I plan to send that for DOB proof along with Non-Availabiltiy of birth certificate.
Because Non-Availability was made based of the passport info, it does not have the last name either....
His father (My grand father) is no more alive.
So Qs is, what should I fill in the Family Name of my father's father details...should I leave it blank or fill it.
