DS-230 (Last Questions Befor Sending )

Hi

I completed all of my forms and it is ready to send ... but i still facing problems with the following issues:

1. DS-230- Q10,11 : What if i dont know any address in US ??What will i do?

2. Q21: Persons in 14 and 19 who will accompany you to US , am i have to list all of my family members names who will go with me to US now??

3. Q29: Name and Address of Petitioner???? i'm the one who won the Lottery ..will it be my name in my form and my wife and my baby form??please explain this question for me.

4. My baby is 5 months old i'll sign for her is it ok???

Please help me as soon as possible

For now, you can put "Will let you know at the interview". However, you should have an address at the interview.
