DS-230 & I-864 for minor

Few questions:
- I am filing for a minor (my child) and we need to fill DS-230. Since she is only 10 yrs old, who should sign the form (SIGNATURE OF APPLICANT)
- I am a self employed and I am working on as contractor.
- Since I don't have a company name, what should be my "company name " per Part 4, A.2.
- What should be a reasonable proof of evidence (I-864) for self-employment.

Parents, legal guardians and persons granted power of attorney to act on behalf of a parent or legal guardian, can complete this form on behalf of a minor child. (I found this in the internet, you must submitt the documents that support your parenthood, guardian or others documents that grant you power over the child).

Recent pay statement, type of work performed, salary or wages paid.
