DS-230: Have you ever applied for a SSN?

I have an SSN that was issued to me when I worked in the US which says only valid with work permit. On entry 43a of DS-230, what do I say for "Would you like to receive a replacement card?"

If I say YES, then would I be issued an SSN Card without "valid with work permit" ?

If I say NO, then am I stuck with SSN which has "valid with work permit"?
