DS- 230 Form previous trips to the US

I can't remember the dates and durations of previous trips to the US for the DS-230 form. Can anyone advise how accurate this needs to be? Also there are only 3 lines in that section on the form, does this mean I only write 3 trips?

I had lots of trips to the US so I just wrote "See attached" and added a spreadsheet to my forms. I couldn't remember the long ago trip dates either so I just put month/year for all the trips. They didn't have a problem with that because I was not questioned for more detail and I got my Green Card.

You might also look at your passport. You most likely have an immigration stamp that shows your arrival date in the US.
