The item #25 in the form reads like this:

"list dates of all previous visits to or residence in the united states"

Now, does it need the list of visits or the list of residence in US? or both? This is really confusing. The table has from/to dates, location, type of visa and A# (if any).

I came to US and landed in chicago. Then shifted to CA and finally I came to Virginia. Last year I went to India and came back. Now how should I list these here?

Any help is really appreciated

Each of the moves you made would appear on a line, as follows

MM/YY to MM/YY - Chicago/ IL - (visa Category & visa # )
MM/YY to MM/YY - City / CA - ( " could be same as
above)
MM/YY to (when you left for India) - City / VA - ( " " )

(when you came back) to (current date) city/ State - ( " )

So, it is something like this:

07-1998 08-2000 chicago IL H1B [landed 1st time]
09-2000 12-2002 cupertino CA H1B
01-2002 04-2002 washington DC H1B [left US]
05-2002 present washington DC H1B [landed 2nd time]

or like this:

07-1998 04-2002 chicago / san francisco / DC H1B
04-2002 present washington DC H1B

Either way it is fine. You can break out a new line when the
period changes ( or the H1 changes ). So the time periods you leave out will be your time away from US.
