DS 156 - Questions - Clarifications Requested

I would like to start this thread, as I was unable to find a thread where questions regarding DS - 156 are answered.

I would really appreciate if you can provide your feedback on the questions posted below:

Q 8 - Other Surnames Used
Leave Blank or NONE or NA

Q 9 - Other First and Middle Names Used
Leave Blank or NONE or NA

Q 15 - Home Address
Permanent Address outside US or Current US Address (F1- OPT - H1 Currently in US)

Q 21 - Present Occupation
Temporary Worker or Software Engineer or Programmer Analyst (Designation mentioned on Offer Letter)

Q 22 - When do you intend to arrive in US
Nov 9 2006 (My appointment date) - Assuming I will get the visa the same day ??

Q 25 - Name and Tel No's of Person in US Who You will be staying with or visiting for Tourism or Business -
Do I need to provide my current address as I would come back here and my rommated name. Confused !!

Q 26 - How long do you Intend to stay in US -
Date mentioned in 797 approval, I assume.

Q 27 - What is the Purpose of your Trip? - Temporary Employment ?? !!

Q 28 - Who will pay for your trip? -
Myself / Temporary Employment / Employer ?? !!

Q 29 - Have you ever been in US - YES -

Most Recent When ?? - Last Entry to US date to be mentioned !!

NOTE: Please excuse me if I am posing some very obvious questions. Just want to be sure to provide correct information in these forms.

All the questions are easier to answer for someone visiting the country for the first time (Like my parents who recently visited me). I have doubts as what to answer if I am already working here in US.
 
Q8 and Q9: Blank.

Q15: If you are already in the US and plan to come back to the US after visa stamping, then current address in the US.

Q21: Use designation used in offer letter - always good to have information corroborated by other documentation.

Q22: Not sure where you are getting the visa from, but in Canada you will not get the visa the same day. Use a date corresponding to a day or two after the appointment date.

Q25: Since you will be coming back on H1, use the company's name, as well as the employer's main phone number - should be sufficient.

Q26: Correct assumption.

Q27: Employment.

Q28: "My Employer".

Q29: When? Date of last entry into the US. For How Long: either "Current date" if you are filling out the form in the US, or whatever date you will leave US to get the visa. You may also have to specify other date ranges that you have been in the US before. Enter the dates chronologically backwards.

I believe there is an online form DS-156 that the consulates recommend you to fill out. This online form generates a PDF file with a barcode that makes it easier for the consulate to scan/lookup your data. If you fill this form out online while in the US, you can fill in data that is true as of the time you are filling it out. For example, Q29, "For how long -> till current date".
