DS-156 Clarifications

Hello,

While filling the DS-156, i got some doubts, can some one clarify me on below:

33 - Has anyone ever filed an immigrant visa petition on your behalf? (my i140 got approved a week back, is it a immigration petiton and yes, then shall i mention my labour filing date or I-140 filed date?

40 - List all countries you have entered in the last ten years (Give the year of each visit) then select Add Country (selection will not be saved until the Add Country button is pressed) - I visited india once for 20days, so shall i mention two times i entered into us with dates?

#33 Your I-140 is the immigrant petition ... so the answer is yes. I don't think you have to mention the date of application.
#40 - yes do that.

will approved I-140 has any impact on h1b extension stamping. it's my 1st enxtension.

That won't be necessary unless you are revalidating the visa for 3 years based on I-140 approval. In that case ask your employer for the copy of it.

I am revalidating for one year. I filed 7th year extension based on pending I-140 application.

In that case take any document that states your priority date - please note that your PD should be a year ahead of the H-1B 6th year expiry date. Furnish the document only if asked for.
