ds-156 and canadian landing

I am planning to land next week. I need to get the h1-b stamped as well
for the stamping they are asking me to fill the ds-156 form in which it is asking for permanent address. I think it is obviously my current US address.
Please advice if it is wrong or I need to give the Canada address
