Doubt in filling I-765 FORM,

Hello,

I got a doubt on one of the fields in I-765 form.I am going to file all by myself this time, Hence ur opinions matter.

For Question 11, Have u ever applied for EAD before?

Which uscis office and Date.

For the Date, what should I put in. Should I put the dates when my current ead is valid or just put the starting date in my EAD card.

I would put the date when I applied for EAD last time.
