Does USCIS mail to both Applicant & Attorney ?

Simple question:

For any mail sent by USCIS ( examples being RFE's, Status Inquiry Response ),
is it always sent to both applicant as well as attorney ?

Or is there a possibility that it goes only to applicant and not to
attorney ?

Reason for my asking is that I think USCIS' is a major screw-up with
respect to address change of applicant; and then if stuff like RFE
comes only to applicant and with a screwed up address, then its a
big-time mess for the applicant.

At least a delivery to an attorney guarantees that RFE's don't get
overlooked.

I am not an expert, but as far as I know,

140 approval notice - goes to attorney
FP notification - goes to both
RFE - goes to attorney
address change acknowledgement - goes to the applicant
485 approval notice - goes to both
status inquiry acknowledgement - goes to the applicant (even when the attorney initiates the inquiry)
