Here is the CASE.

My friends wife is the primary applicant and my friend is beneficiary.
Their GC was recently approved and my friends wife wants to take a
indefinite break from job to take care of kids since my friend is working.
Since their GC is employment based does the primary applicant always has to work?

There is absolutely no problem from sponspering employer for her to leave the job.
 
Yes

The Green Card was granted on the assumption that the primary applicant would work, and work for the sponsoring employer. If one or more of the derivative GC holders works, is irrelevant.

I would say that she should work there for at least 3 months. 6 months is much better. If she has vacation, she can use that.

I believe if one takes leave of absence, one is still considered to be employed. You can check on this one in detail with someone. I am not 100% sure on this.
