I have the same question. I heard somewhere that the medicals are valid for a year. Knowing how the VSC is processing the I-485 applications, will the medicals be invalid after a year? Please post. People whose I-485 applications are currently being processed (Sep 99 and Oct 99 NDs) please post.

I guess you have to go to the doctor (INS approved) again and ask them to give you a new immunization certificate. Doctor will check your records and see if there is any new kind of vaccine that is required under latest vaccination requirements. If yes, then you will take it. Otherwise, they will just prepare the paperwork saying everything is done and send it/give it to you in a sealed envelope. I guess INS just wants to make sure, you are up-to-date with any vaccination requirements.
