Any one need to get registered must need to do that. Now if you were already registered once & went out of US then when you will re-enter US the immigration officer at POE will advise you to go for registration or not. Else after completion of one yr you need to go again for registration.
They register you when you re-enter back in USA. They take ur pIc/fingure prints etc tec.
